What do I want to accomplish? (life)

As of right now, I'm not sure what exactly I'd like to accomplish. I do know that I want to be successful in whatever I do. If I take a few wrong turns and end up working at McDonald's, I want to be the best McDonald's employee the world has ever known. This ties into my personal core value of accomplishment. 
I know for sure that I want to be a leader who leads with integrity. Integrity is highly important to me because it's what represents someone as a person in general. If I lack in integrity, then why would anyone take what I have to say seriously?
I also know that I want to inspire others in the upcoming future. Because of my concern for others, I want to change the lives of others around me for the better. Helping others to accomplish their goals brings me satisfaction and helps me feel like I have actually accomplished something good for the world.
My concern for others is the reason why I have chosen to pursue a career in medicine. After working in the United States for a while (saving money), I want to work in a third-world country to give my service to those who need it most (but can't afford to pay for it). If I had an money weren't a scarcity, I would use it to bring health to communities across the world, at my own expense. Health is something that every creature of this Earth deserves to have.
Overall, as a leader, I want to inspire others, give time and money to others, and do all of that with integrity. I want to be a leader who does these things because I care, not because I think it will make me a "better" person.
